Key Responsibilities & Duties

  • Conduct thorough patient assessments, including screenings, diagnoses, and treatments, ensuring comprehensive care.
  • Provide compassionate, unbiased care aligned with evidence-based guidelines and best practices.
  • Coordinate patient care within a team-based model to optimize outcomes and satisfaction.
  • Ensure timely follow-up on patient needs, including test results and referrals.

Job Requirements

  • Bachelor of Science in Nursing (BSN) or equivalent medical degree (MD/DO).
  • Active New York State Certification/Licensure (MD, DO, or NP) and current registration.
  • DEA Controlled Substances Registration Certificate and current CPR Certification.
  • Board certification or eligibility in Family Medicine, Internal Medicine, or Infectious Disease.
  • Minimum of 1 year of experience in primary care; 2-3 years preferred.
  • Experience in HIV and transgender care is highly desirable.
  • Strong collaboration skills and commitment to accessible, patient-centered care.
  • Bilingual proficiency (Spanish/English) is advantageous.
