Job Description

Overview

  • Perform comprehensive assessments using the Uniform Assessment System (UAS-NY) to evaluate members' clinical, psychological, and social needs.
  • Ensure compliance with New York State Department of Health regulations and maintain accurate documentation of assessments.
  • Collaborate with interdisciplinary teams to develop personalized care plans that address members' unique requirements.
  • Conduct hybrid assessments, including telephonic and in-home visits, to accommodate member preferences and needs.
  • Complete enrollment and reassessment cases within established timeframes and organizational policies.
  • Provide clear explanations of program benefits and rules to members, ensuring accessibility and understanding.
  • Offer training and support during onboarding to prepare for field assessments effectively.
  • Maintain open communication with members, families, caregivers, and healthcare professionals regarding care plans and progress.

Key Responsibilities & Duties

  • Conduct initial enrollment assessments and periodic reassessments using UAS-NY protocols to determine member eligibility and care needs.
  • Develop and implement safe and effective care plans tailored to individual member requirements.
  • Communicate assessment outcomes to relevant parties, including referral sources and enrollment representatives.
  • Ensure timely completion of all documentation, adhering to regulatory guidelines and organizational standards.
  • Address urgent requests and safety concerns promptly to ensure member well-being.
  • Collaborate with care management teams to provide comprehensive support and guidance to members.
  • Educate members on program rules, benefits, and responsibilities to facilitate informed decision-making.
  • Assist members in navigating healthcare services and understanding Medicare benefits.

Job Requirements

  • Associate Degree in Nursing required; Bachelor's Degree in Nursing preferred.
  • Active RN license with at least one year of experience in community health or managed care.
  • Proficiency in Microsoft Office applications and familiarity with UAS-NY systems.
  • Bilingual skills in Bangla, Hindi, Urdu, Mandarin, Cantonese, or Spanish are advantageous.
  • Ability to manage primary and secondary territories for conducting assessments effectively.
  • Comfortable performing hybrid assessments, including telephonic and in-home visits.
  • Strong organizational skills to handle multiple cases and meet productivity goals.
  • Experience with MLTC or homecare assessments is highly desirable.
