Overview

  • Serve as a Hospice Case Manager (RN), delivering compassionate care to patients in their homes and communities.
  • Coordinate and implement individualized care plans, ensuring optimal health outcomes for patients and families.

Key Responsibilities & Duties

  • Conduct initial and ongoing assessments to determine patient needs and develop care plans.
  • Administer medications and treatments as prescribed, ensuring adherence to care protocols.
  • Provide health education and counseling to patients and families to support care goals.
  • Coordinate with physicians and healthcare teams to address patient conditions and update care plans.
  • Document patient care activities accurately and timely using prescribed tools.
  • Supervise home health aides, ensuring quality and compliance in care delivery.
  • Participate in staff meetings and contribute to team development and knowledge sharing.
  • Engage in discharge planning to ensure seamless transitions for patients.

Job Requirements

  • Graduate of an accredited nursing program; Bachelor’s degree in Nursing preferred.
  • Current RN licensure in the state and valid CPR certification.
  • Minimum of one year of acute care or home care nursing experience.
  • Strong communication, problem-solving, and observational skills.
  • Ability to supervise home health aides and coordinate interdisciplinary care.
  • Physical capability to perform nursing duties, including lifting and transferring patients.
  • Proficiency in computer usage for documentation purposes is advantageous.
