Key Responsibilities & Duties
- Conduct initial and ongoing assessments to determine patient needs and develop care plans.
- Administer medications and treatments as prescribed, ensuring adherence to care protocols.
- Provide health education and counseling to patients and families to support care goals.
- Coordinate with physicians and healthcare teams to address patient conditions and update care plans.
- Document patient care activities accurately and timely using prescribed tools.
- Supervise home health aides, ensuring quality and compliance in care delivery.
- Participate in staff meetings and contribute to team development and knowledge sharing.
- Engage in discharge planning to ensure seamless transitions for patients.
Job Requirements
- Graduate of an accredited nursing program; Bachelor’s degree in Nursing preferred.
- Current RN licensure in the state and valid CPR certification.
- Minimum of one year of acute care or home care nursing experience.
- Strong communication, problem-solving, and observational skills.
- Ability to supervise home health aides and coordinate interdisciplinary care.
- Physical capability to perform nursing duties, including lifting and transferring patients.
- Proficiency in computer usage for documentation purposes is advantageous.
