Overview
- Provide skilled nursing services within a certified home health agency, ensuring safe and effective care in patients' homes.
- Collaborate with patients, families, and physicians to develop and implement personalized care plans.
- Conduct clinical assessments to evaluate patient needs and home care safety.
- Monitor and adjust care plans as necessary to meet evolving patient requirements.
- Provide oversight and guidance to Home Health Aide services for optimal patient support.
- Participate in interdisciplinary care delivery processes to ensure comprehensive patient care.
- Utilize bilingual skills to enhance communication and care delivery, if applicable.
- Engage in professional development opportunities to maintain and enhance nursing expertise.
Key Responsibilities & Duties
- Conduct initial and ongoing patient assessments to determine home care needs and safety.
- Develop, oversee, and implement individualized care plans in collaboration with interdisciplinary teams.
- Supervise Home Health Aides to ensure adherence to care plans and patient safety.
- Monitor patient progress and adjust care plans as necessary to address changing conditions.
- Perform Patient Review Instrument (PRI) assessments to evaluate patient needs.
- Maintain accurate documentation of patient care activities and outcomes.
- Ensure compliance with all regulatory and organizational standards in care delivery.
- Provide education and support to patients and families regarding care plans and health management.
Job Requirements
- Current New York State RN license and registration required.
- Associate's Degree in Nursing required; Bachelor's Degree in Nursing preferred.
- Minimum of 2 years of experience in home care assessments and care plan development.
- At least 1 year of supervisory experience in a hospital or home care setting.
- Proficiency in Patient Review Instrument (PRI) assessments preferred.
- Valid driver’s license and ability to travel within the Manhattan area.
- Proficiency in Microsoft Office applications for documentation and communication.
- Bilingual skills are advantageous for effective patient and family communication.
