Key Responsibilities & Duties

• Conduct case management for patients, ensuring optimal care and discharge planning.
• Monitor up to two units during weekends, focusing on patient transitions.
• Collaborate with multidisciplinary teams to address patient needs and goals.
• Maintain accurate and timely documentation of patient interactions and plans.
• Adhere to facility protocols and state licensing requirements for social work practice.
• Provide support during holiday coverage, ensuring continuity of care.
• Engage in professional development to enhance skills and knowledge.

Job Requirements

• Master of Social Work degree from an accredited institution.
• Minimum of two years of acute hospital case management experience.
• Active California state license or pending license acceptance.
• Ability to work every other weekend and cover holidays as needed.
• Proficiency in discharge planning and patient care coordination.
• Strong communication and collaboration skills within a healthcare team.
