Overview

Key Responsibilities & Duties

  • Diagnose and treat acute and chronic conditions in older adult patients.
  • Develop and implement personalized care plans to achieve patient health goals.
  • Lead and supervise a care team, ensuring effective collaboration and communication.
  • Engage with hospitalists and other specialists to coordinate patient care.
  • Provide education and counseling to patients on health promotion and disease prevention.
  • Document patient interactions and care plans accurately using electronic health records.

  • Ensure compliance with medical guidelines and quality standards in patient care.

Job Requirements

  • Doctor of Medicine (MD) degree with board certification in Internal Medicine, Family Medicine, or Geriatrics.
  • Active state medical license and DEA certification for controlled substances.
  • Minimum of 1 year of experience, with 5 years preferred in primary care settings.
  • Proficiency in electronic health record systems and Microsoft Office Suite.
  • Strong leadership and communication skills to guide care teams and patient interactions.
  • Commitment to continuous professional development and quality improvement.
  • Ability to travel locally for patient care and nationally for training or recruitment.
  • Basic Life Support (BLS) certification within 90 days of employment.
