Job Description

Overview

  • Lead the establishment and operation of a Children's Health Home program, ensuring compliance with Medicaid regulations and delivering high-quality care coordination services.
  • Develop workflows, policies, and procedures tailored to children, youth, and families, adapting proven systems from existing adult programs.
  • Collaborate with senior leadership to achieve program growth, readiness milestones, and long-term sustainability goals.
  • Oversee daily operations, including enrollment, care assignment, and service delivery, ensuring benchmarks are met.
  • Implement quality management systems and ensure compliance with Medicaid Children’s Health Home regulations.
  • Recruit, supervise, and mentor program staff, fostering a culture of accountability and continuous improvement.
  • Build partnerships with networks, providers, and organizations to promote effective cross-system coordination.
  • Represent the organization in external forums, advocating for children and families with complex needs.

Key Responsibilities & Duties

  • Design and implement workflows, policies, and procedures for the Children’s Health Home program.
  • Ensure program structure aligns with NYS DOH standards, including eligibility, consent, and care planning.
  • Monitor caseloads, service utilization, and outcomes to meet regulatory benchmarks.
  • Identify operational gaps and implement process improvements as the program scales.
  • Conduct chart reviews, audits, and corrective action plans to maintain quality assurance.
  • Recruit, train, and mentor care managers and program staff to meet performance expectations.
  • Collaborate with stakeholders and networks to enhance service delivery and coordination.
  • Prepare for and lead internal and external audits, ensuring compliance with regulations.

Job Requirements

  • Master’s degree in Social Work, Public Health, or related field; clinical licensure preferred.
  • Minimum 3 years of program management experience serving children and families with complex needs.
  • At least 2 years of supervisory experience within Medicaid Health Home or similar models.
  • Strong familiarity with Medicaid systems, Children’s Health Home models, and EHR platforms.
  • Proven ability to translate policy and regulation into practice effectively.
  • Comfort leading in a start-up or growth environment, with bilingual Spanish skills as a plus.
  • Excellent organizational and operational leadership skills to drive program success.
  • Strong communication skills to engage with staff, partners, and leadership effectively.
