Overview
- Lead and manage the case management department to ensure compliance with policies and regulatory requirements.
- Coordinate patient care and discharge planning processes across hospital departments and external agencies.
- Oversee daily operations, achieving performance outcomes through interdisciplinary collaboration.
- Develop and implement sustainable performance improvement activities based on data analysis.
- Ensure adherence to care coordination and discharge planning processes for quality and documentation standards.
- Build relationships with key stakeholders, including medical and administrative leaders.
- Adapt to changes and manage multiple priorities effectively in a dynamic environment.
- Promote staff development and competency through onboarding and training initiatives.
Key Responsibilities & Duties
- Direct departmental operations, including staffing, resource allocation, and competency development.
- Evaluate and improve operational processes to meet productivity and quality goals.
- Collaborate with CFO and Division CM Leader to achieve departmental objectives.
- Monitor performance data and implement improvement strategies for optimal outcomes.
- Ensure compliance with organizational policies and external regulatory standards.
- Establish effective working relationships with hospital leaders and external stakeholders.
- Oversee care coordination and discharge planning processes for timeliness and quality.
- Demonstrate executive presence and communication skills in leadership roles.
Job Requirements
- Bachelor of Science in Nursing (BSN) required; Master’s degree preferred.
- Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW) licensure required.
- Minimum of 3 years’ experience in acute care hospital case management required.
- Preferred 2+ years’ experience in case management leadership roles.
- Proven skills in performance improvement, team building, and change management.
- Strong communication, presentation, and motivational capabilities.
- Ability to adapt quickly to change and manage multiple priorities effectively.
- Executive presence and ability to lead interdisciplinary teams.
