Job Description

Overview

  • Provide social work services to participants and caregivers in a supportive environment.
  • Work within an Enhanced Memory Impaired program focusing on dementia care.
  • Flexible working hours, including alternating Saturdays, offering work-life balance.
  • Collaborate with a multidisciplinary team to ensure comprehensive care.
  • Opportunity to utilize bilingual skills, with Spanish proficiency being advantageous.
  • Contribute to assessments and individualized service plans for participants.
  • Assist participants and caregivers in accessing community resources.
  • Coordinate discharge plans and ensure smooth transitions for participants.

Key Responsibilities & Duties

  • Conduct assessments and develop individualized service plans for participants.
  • Provide emotional and practical support to participants and caregivers.
  • Assist in accessing community resources and services for participants.
  • Maintain accurate documentation of services provided and participant progress.
  • Collaborate with the team to ensure comprehensive care delivery.
  • Coordinate discharge plans and facilitate smooth transitions for participants.
  • Participate in program development and improvement initiatives.
  • Support the Enhanced Memory Impaired program with specialized dementia care expertise.

Job Requirements

  • Master of Social Work degree and Social Worker License required.
  • Minimum of 1 year experience, with 5 years preferred, in dementia care.
  • Certified in dementia care is a plus, showcasing specialized expertise.
  • Bilingual proficiency in Spanish is advantageous for effective communication.
  • Strong documentation and organizational skills for maintaining accurate records.
  • Ability to work on-site with flexible hours, including alternating Saturdays.
  • Excellent interpersonal skills to support participants and caregivers effectively.
