Field Nurse

Overview

• Deliver personalized nursing care to patients in a home healthcare setting, promoting their health and recovery through expert clinical practices.
• Educate patients and families on health management strategies, fostering understanding and adherence to preventive care measures.

Key Responsibilities & Duties

• Conduct thorough patient assessments, including OASIS evaluations, to determine individualized care requirements.
• Develop and update Physician Plans of Care collaboratively with healthcare professionals.
• Perform essential home health services such as admissions, re-certifications, and discharges.
• Ensure compliance with healthcare regulations and standards in all patient care activities.
• Document patient care activities accurately and efficiently using electronic health record systems.
• Provide culturally sensitive care to diverse patient populations, respecting individual needs and preferences.
• Travel to patient locations to deliver high-quality care and maintain service accessibility.
• Participate in professional development opportunities to stay current in clinical practices.

Job Requirements

• Associate of Applied Science (AAS) in Nursing or equivalent educational qualification.
• Active nursing license with demonstrated experience as a registered nurse.
• Experience in pediatric and acute care settings is highly preferred.
• Proficiency in completing OASIS assessments and maintaining accurate documentation.
• Familiarity with Homecare Homebase (HCHB) software is advantageous.
• Ability to drive and access a reliable vehicle for patient visits.
• Strong communication and interpersonal skills for effective patient and family interaction.
