Job Description

Overview

  • Provide primary and preventative care for patients in skilled nursing facilities.
  • Manage residents experiencing acute changes in condition and chronic illnesses.
  • Prescribe medications, including controlled substances, adhering to state regulations.
  • Collaborate with interdisciplinary teams to coordinate comprehensive patient care.
  • Utilize electronic medical records (EMR) for accurate documentation and patient management.
  • Conduct advanced care planning and chronic care management for residents.
  • Communicate regularly with nursing home staff to address patient care needs.
  • Order and review diagnostic tests and treatments, ensuring appropriate follow-up.

Job Requirements

  • Doctorate of Nursing Practice (DNP) degree required.
  • Current, unrestricted Nurse Practitioner license in the state of practice.
  • Board Certification and DEA licensure mandatory.
  • Experience in adult, family, or geriatric specialties preferred.
  • Proficiency in managing patients with advanced illnesses and chronic conditions.
  • Familiarity with EMR systems for efficient patient management.
  • Ability to work collaboratively within interdisciplinary care teams.
