Overview
- Provide clinical support and leadership for hospice and palliative care services within a healthcare organization.
- Evaluate patients referred for hospice or palliative care, ensuring compliance with laws and regulations.
- Facilitate patient transitions into hospice care, coordinating interdisciplinary team efforts.
- Engage in community outreach and education to promote hospice services.
- Develop and expand hospice programs in community and facility-based settings.

Key Responsibilities & Duties
- Evaluate and assess patients for hospice and palliative care services.
- Coordinate interdisciplinary team resources for comprehensive patient care.
- Provide clinical oversight and support for hospice program development.

- Plan and execute clinical marketing strategies for service area growth.

Job Requirements
- Bachelor of Science in Nursing (BSN) from an accredited institution.
- Minimum of two years of experience in Medical-Surgical or Community Health nursing.
- Preferred experience in Hospice or Oncology nursing.
- New York State RN licensure, CPR-BLS certification, and valid NYS Driver's license preferred.
- Proficiency in Microsoft Office, including Word and Excel.
- Strong communication, teaching, and listening skills.
- Ability to work non-judgmentally with diverse patient populations.
- Demonstrated critical thinking and public speaking abilities.
