Job Description

Overview

  • Provide expert social work services emphasizing discharge planning and Medicare compliance.
  • Work on-site Friday through Monday, completing 10-hour shifts.
  • Utilize advanced systems for documentation and workflow management.
  • Collaborate with healthcare teams to ensure seamless patient transitions.
  • Engage with patients and families to address social and emotional needs.
  • Maintain compliance with Medicare regulations, including MOONs and IMMs.
  • Participate in professional development to enhance skills and knowledge.
  • Ensure accurate and timely documentation of interventions and plans.

Key Responsibilities & Duties

  • Develop individualized discharge plans to meet patient needs effectively.
  • Coordinate with multidisciplinary teams to optimize care transitions.
  • Prepare and manage Medicare compliance documentation accurately.
  • Utilize advanced systems for patient documentation and workflow.
  • Provide counseling and support to patients and families during transitions.

Job Requirements

  • Master of Social Work (MSW) degree required; LMSW or LCSW certification preferred.
