Overview

  • Deliver exceptional primary care services to patients in skilled nursing facilities, focusing on acute and chronic conditions.
  • Collaborate with interdisciplinary teams to ensure comprehensive and patient-centered care plans.
  • Utilize advanced care planning strategies to optimize patient health outcomes and quality of life.
  • Prescribe medications and treatments in compliance with state regulations and best practices.
  • Document patient interactions and care plans accurately using electronic medical records (EMR).

  • Communicate effectively with patients, families, and facility staff to address healthcare needs.
  • Order and interpret diagnostic tests, ensuring timely follow-up and treatment adjustments.

Key Responsibilities & Duties

  • Provide comprehensive medical care to long-term care residents, addressing acute and chronic health conditions.
  • Develop and implement individualized care plans in collaboration with interdisciplinary teams.
  • Prescribe medications and therapies while adhering to regulatory guidelines and standards.
  • Perform advanced care planning, including chronic disease management and preventative strategies.
  • Utilize EMR systems for accurate documentation and efficient patient management.

Job Requirements

  • Doctorate of Nursing Practice (DNP) degree is required for this role.
  • Current, unrestricted Nurse Practitioner license in the state of practice.
  • Board Certification and DEA licensure are mandatory qualifications.
  • Experience in adult, family, or geriatric specialties is highly preferred.
  • Proficiency in managing patients with complex chronic conditions and advanced illnesses.
  • Familiarity with EMR systems for efficient documentation and patient tracking.
  • Strong collaboration skills to work effectively within interdisciplinary teams.
