Job Description
Overview
- Provide essential nursing care and support to patients in a long-term care setting, ensuring their safety and satisfaction.
- Work under the supervision of a Registered Nurse or Clinical Care Manager, following established protocols and procedures.
- Perform duties as a Licensed Practical Nurse (LPN), including medication administration and patient monitoring.
- Participate in a rotating weekend schedule to ensure consistent patient care.
- Complete a comprehensive four-day orientation program to familiarize with organizational practices.
- Engage in in-person interviews to demonstrate qualifications and suitability for the role.
- Commit to a consistent work schedule from 3:00 PM to 11:00 PM.
- Ensure compliance with all regulatory and organizational standards for patient care.
Key Responsibilities & Duties
- Administer medications and treatments as prescribed, ensuring accuracy and adherence to protocols.
- Monitor patient conditions and report changes to the supervising nurse or physician.
- Maintain accurate and detailed patient records, documenting care provided and observations.
- Assist patients with daily living activities, promoting comfort and well-being.
- Collaborate with interdisciplinary teams to develop and implement patient care plans.
- Provide emotional support and education to patients and their families regarding care plans and procedures.
- Ensure compliance with infection control and safety protocols within the care environment.
- Participate in ongoing training and professional development opportunities to enhance skills.
Job Requirements
- Valid NYS Licensed Practical Nurse (LPN) license required.
- Minimum of two years of experience in long-term care or geriatric nursing.
- Graduate of an approved practical nursing program with BCLS and IV certification.
- High school diploma or GED equivalent required.
- Commitment to working every other weekend as part of the schedule.
- Strong communication and interpersonal skills to interact effectively with patients and staff.
- Ability to work on-site during designated hours of 3:00 PM to 11:00 PM.
- Completion of a four-day orientation program upon hiring.
