Overview

Key Responsibilities & Duties

• Conduct comprehensive assessments to identify patient needs and develop appropriate discharge plans.

Job Requirements

• Master of Social Work degree from an accredited institution is required.
• Minimum of 2 years of acute hospital case management experience is essential.
• Active California state license or pending license is mandatory.
• Ability to work every other weekend and provide holiday coverage as needed.
• Strong interpersonal skills to effectively communicate with patients, families, and healthcare teams.
• Knowledge of discharge planning processes and community resources.
