Key Responsibilities & Duties

• Supervise assigned staff and ensure adherence to medical protocols.
• Coordinate client medical and psychosocial needs through referrals and treatment suggestions.
• Conduct health and vitals checks to manage client health effectively.
• Document findings and maintain medication administration records accurately.
• Provide health education and advocate for client health preferences.
• Participate in treatment team meetings and case conferences.
• Deliver emergency first aid and CPR assistance as required.
• Assist clients with practical needs, enhancing their daily living skills.
• Support client goal development through collaborative partnerships.

Job Requirements

• Active LPN license required, with a minimum of 1 year of experience.
• Preferred 3 years of experience in medical or psychiatric care settings.
• Associate of Science (AS) degree in nursing or related field.
• Strong problem-solving skills and ability to manage complex client cases.
• Proficiency in health education and advocacy for diverse populations.
• Experience with documentation and medication record maintenance.
• Capability to work collaboratively in a multidisciplinary team environment.
