Overview

  • Serve as a Hospice Care Planner, facilitating patient transitions from hospital to home care services.
  • Collaborate with healthcare professionals to develop comprehensive care plans tailored to patient needs.

Key Responsibilities & Duties

  • Evaluate patient needs and establish individualized care plans for hospice services.
  • Coordinate patient admissions to home care programs, ensuring smooth transitions.
  • Collaborate with interdisciplinary teams to optimize patient care outcomes.
  • Develop and maintain relationships with healthcare professionals to support patient referrals.
  • Conduct physical assessments and interviews to determine patient eligibility for hospice care.

Job Requirements

  • Bachelor of Science in Nursing (BSN) required; advanced degree preferred.
  • Active RN license with current registration in New York State.
  • Minimum of two years of experience in hospice, critical care, oncology, or ICU settings.
  • Proficient in physical assessment and patient interviewing techniques.
  • Strong communication and problem-solving skills essential for independent work.
  • Experience in sales or business development is preferred but not required.
  • Client-service focus with excellent written and verbal communication abilities.
  • Availability for occasional weekend shifts and in-office training sessions.
