Job Description

Overview

  • Provide comprehensive primary and preventive care to adult patients in a culturally diverse community-based health center.
  • Diagnose and manage acute and chronic medical conditions while promoting overall wellness and patient education.
  • Collaborate with multidisciplinary teams to ensure coordinated, patient-centered care delivery.
  • Maintain accurate and timely documentation within the electronic health record (EHR) system.
  • Participate in quality improvement initiatives and contribute to team meetings and discussions.
  • Engage with the community to address healthcare needs and promote preventive care strategies.
  • Support the health center's mission by delivering culturally competent care to a diverse patient population.
  • Provide guidance on lifestyle modifications, disease prevention, and medication management.

Key Responsibilities & Duties

  • Conduct physical examinations, screenings, and chronic disease management for adult patients.
  • Diagnose and treat acute and chronic medical conditions, ensuring optimal patient outcomes.
  • Collaborate with multidisciplinary teams to develop and implement patient-centered care plans.
  • Educate patients on lifestyle modifications, disease prevention, and medication adherence.
  • Maintain accurate and timely documentation in the electronic health record (EHR) system.
  • Participate in quality improvement initiatives to enhance healthcare delivery and patient satisfaction.
  • Engage in team meetings and contribute to the development of best practices in patient care.
  • Perform additional tasks as directed by the supervisor to support the health center's operations.

Job Requirements

  • Master of Science in Nursing with specialization in Internal Medicine or Adult Primary Care.
  • Current New York State license or eligibility for licensure is required.
  • Minimum of 2 years of experience in community health or outpatient primary care.
  • Preferred 4 years of experience in Internal Medicine or Adult Primary Care settings.
  • Strong communication and interpersonal skills to effectively interact with patients and teams.
  • Bilingual proficiency in Creole and English is strongly preferred.
  • Ability to work on-site in a community-based health center environment.
