Overview

  • Provide direct support to individuals, promoting independence and enhancing their quality of life through personalized care and assistance.
  • Assist with daily living activities, including personal care, meal preparation, and recreational engagement tailored to individual needs.
  • Coordinate and accompany individuals to medical appointments, ensuring their healthcare needs are met effectively.
  • Maintain accurate documentation of care activities, progress, and appointments for seamless communication with the care team.
  • Foster positive relationships with individuals and their families, creating a supportive and inclusive environment.
  • Ensure a safe, clean, and organized living space, adhering to health and safety standards at all times.

Key Responsibilities & Duties

  • Deliver compassionate care and support to individuals, ensuring their comfort and safety in daily activities.
  • Facilitate medical appointments, providing transportation and advocating for healthcare needs effectively.
  • Assist with personal care routines, recreational activities, and household management tasks to support daily living.
  • Document medical appointments, treatments, and medication administration accurately for effective communication.
  • Build and maintain professional relationships with individuals, families, and healthcare providers to foster collaboration.

Job Requirements

  • High School diploma or GED equivalent is required for this role.
  • Valid Driver's License to operate vehicles is mandatory for transportation tasks.
  • Minimum of one year experience working with OPWDD populations is preferred.
  • Completion of OPWDD Medication Certification, CPR, First Aid, and SCIP-R training is required.
  • Ability to lift up to 50 pounds and assist individuals with mobility needs.
  • Role requires standing and walking for extended periods; non-sedentary work environment.
  • Flexibility to work various shifts, including evenings and overnight schedules.
