Key Responsibilities & Duties

• Diagnose and manage acute and chronic health conditions to achieve optimal patient outcomes.
• Develop and implement personalized care plans in collaboration with patients and interdisciplinary teams.
• Educate patients on preventive care measures and lifestyle changes to promote wellness.
• Effectively document patient interactions and care plans using electronic health records.
• Coordinate with specialists and hospitalists to ensure seamless, comprehensive care delivery.
• Participate in quality improvement initiatives and adhere to evidence-based clinical guidelines.
• Attend training sessions and organizational meetings to stay updated on best practices and goals.
• Lead care team efforts to enhance patient satisfaction and health outcomes.

Job Requirements

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
• Board certification in Internal Medicine, Family Medicine, or Geriatrics is mandatory.
• Active state medical license and DEA certification for controlled substances.
• Minimum of 1 year of experience in primary care; 5 years preferred.
• Proficiency in electronic health records and comfortable utilizing technology in practice.
• Strong commitment to patient-centered care and value-based healthcare principles.
• Excellent communication and leadership skills to guide and inspire care teams.
• Basic Life Support (BLS) certification required within 90 days of employment.
