Job Description

Overview

  • Engage Medicare members, pharmacies, and providers to address adherence and SUPD gaps effectively.
  • Conduct Comprehensive Medication Reviews (CMRs) to enhance members' understanding of their medications and health.
  • Provide clinical guidance and support to Pharmacy Quality Specialists for improved healthcare outcomes.
  • Collaborate with pharmacies and providers to resolve medication adherence challenges.
  • Offer counseling and information to Medicare members regarding pharmacy benefits and medication use.
  • Utilize CMR platforms to alert providers about potential drug interactions and adverse reactions.
  • Ensure compliance with healthcare regulations while delivering high-quality clinical services.

Key Responsibilities & Duties

  • Conduct outreach to Medicare members to address barriers to medication adherence.
  • Partner with pharmacies and providers to close adherence gaps and enhance patient care.
  • Complete CMRs to provide members with insights into their medications and health.
  • Send provider interventions through fax or CMR platforms regarding drug interactions and dose changes.
  • Counsel Medicare members on medication use and provide information about pharmacy benefits.
  • Collaborate with Pharmacy Quality Specialists to improve healthcare delivery.
  • Maintain accurate documentation and reporting for all clinical activities.

Job Requirements

  • Doctor of Pharmacy (PharmD) degree is required.
  • Minimum of 3 years of experience in a related clinical field.
  • Current state pharmacy license is mandatory.
  • Strong knowledge of Medicare benefits and pharmacy regulations.
  • Proficiency in conducting Comprehensive Medication Reviews (CMRs).
  • Excellent communication skills for member and provider interactions.
  • Ability to work independently in a remote setting.
