Overview
- Provide comprehensive intake services for patients, including insurance eligibility verification and registration.
- Ensure accurate completion and submission of sliding fee scale applications for uninsured patients.
- Issue Good Faith Estimates for patients opting for self-pay or alternative insurance.
- Assist with enrollment of uninsured patients in insurance programs through NYS Marketplace.
- Perform outreach activities to engage patients who have not accessed services recently.
- Collaborate with interdisciplinary teams and participate in health fairs for insurance enrollment.
- Support advocacy efforts to address homelessness and AIDS crises.
- Maintain compliance with HIPAA regulations and ensure accurate documentation in EMR.
Key Responsibilities & Duties
- Screen patients for program and insurance eligibility, including ADHC, OASAS, Health Home, and OMH.
- Complete PRAPARE screenings and generate leads for new patients in eICare.
- Assist patients with Medicaid HMO enrollment and insurance changes.
- Document clinical interventions and outreach efforts in the medical record.
- Participate in morning huddles, medical staff meetings, and quality improvement initiatives.
- Perform professional scheduling of patient appointments and registration processes.
- Provide referrals for case management and coordination of care services.
- Understand and practice organizational core values in a fast-paced environment.
Job Requirements
- High School Diploma or GED required; proficiency in Spanish preferred.
- Minimum of 2 years of experience in case management or related field.
- Strong knowledge of HIV/AIDS, entitlements, and psychosocial assessments.
- Ability to discern medical emergencies and alert appropriate personnel.
- Knowledge of HIPAA regulations and proficiency in EMR systems.
- Excellent organizational and clinical skills in a dynamic environment.
- Willingness to work evenings and weekends as required.
- Completion of NYS Assistor Training for Insurance Enrollment within 30 days.
