Key Responsibilities & Duties

• Conduct comprehensive assessments to evaluate member needs and eligibility for services.
• Develop and review personalized service plans in collaboration with members and teams.
• Manage enrollment processes, ensuring adherence to policies and standards.
• Provide guidance and support to members throughout the enrollment phase.
• Engage in ongoing training to enhance proficiency in enrollment tools and procedures.
• Collaborate with interdisciplinary teams to optimize service delivery and outcomes.

Job Requirements

• Bachelor of Science (BS) in Nursing is required.
• Active Registered Nurse (RN) license in the State of New York is mandatory.
• Minimum of 1 year of experience in nursing or enrollment-related roles.
• Proficiency in using assessment tools and processes is preferred.
• Ability to work effectively in a hybrid environment with occasional field visits.
