Overview
- Provide in-home therapy services to enhance communication and swallowing functions.
- Develop and implement individualized treatment plans based on comprehensive evaluations.
- Ensure compliance with physician orders, organizational policies, and regulatory standards.
- Focus on patient-centered care and education tailored to individual needs.
- Collaborate with interdisciplinary team members to coordinate care and referrals.
- Maintain accurate and timely documentation in electronic medical records (EMR).
- Monitor patient progress and adjust treatment plans as needed.
- Adhere to organizational competencies and professional standards.
Key Responsibilities & Duties
- Conduct comprehensive evaluations in the home setting within required timeframes.
- Deliver skilled speech-language pathology services per physician’s plan of care.
- Educate patients and caregivers on communication strategies and home programs.
- Collaborate with physicians and team members for effective care coordination.
- Participate in discharge planning throughout the course of care.
- Maintain productivity standards in a home health setting.
- Document all case communication securely and accurately.
- Demonstrate flexibility and perform additional duties as assigned.
Job Requirements
- Master’s Degree in Speech-Language Pathology is required.
- Active Speech Language Pathologist License (NJ) is mandatory.
- AHA Basic Life Support (BLS) Certification is essential.
- Minimum of 6 months experience in Speech Language Pathology.
- Proficiency in basic computer operations and electronic medical records (EMR).
- Ability to lift, push, and pull up to 50 lbs.
- Capability to stand for extended periods (up to 8 hours/day).
