Overview
- Serve as a vital liaison in patient intake processes, ensuring seamless communication and coordination for home care services.
- Facilitate intake operations by managing referrals and documentation from hospitals, physicians, and community sources.
- Ensure compliance with HIPAA regulations and uphold confidentiality in all interactions.
- Collaborate with clinical, administrative, and billing teams to streamline patient onboarding.
- Provide administrative support and resolve intake-related issues promptly and effectively.
- Track and analyze intake metrics to identify areas for operational improvement.
- Utilize specialized systems for data management, ensuring accuracy and efficiency in patient records.
- Contribute to organizational programs by managing patient onboarding processes effectively.
Key Responsibilities & Duties
- Oversee the intake process for new patients, ensuring accuracy in demographic and insurance data collection.
- Conduct intake interviews and gather required documentation efficiently and professionally.
- Follow up on incomplete referrals and pending documents to ensure timely onboarding.
- Handle high call volumes, addressing inquiries and resolving issues effectively.
- Maintain strict adherence to HIPAA regulations and confidentiality standards.
- Collaborate with cross-functional teams to ensure a seamless patient onboarding experience.
- Provide general administrative support to enhance operational efficiency.
- Escalate complex cases and intake issues to appropriate departments for resolution.
Job Requirements
- Bachelor of Science (BS) degree is required for this role.
- Minimum of 1 year of experience in home care intake; 3 years preferred.
- Proficiency in English and Spanish, both spoken and written, is mandatory.
- Experience with systems like HHA Exchange and familiarity with ePaces or Zoho is advantageous.
- Knowledge of Medicare and Medicaid regulations is highly beneficial.
- Strong data entry skills and attention to detail are essential for success.
- Proficiency in Microsoft Office and standard office systems is required.
