Overview

- Serve as a key resource for clients, connecting them with healthcare providers and community resources.

Key Responsibilities & Duties
- Manage a caseload of clients, coordinating medical appointments and supportive services.
- Conduct routine home visits to assess client needs and provide ongoing support.

Job Requirements
- Active New York State Licensed Practical Nurse (LPN) license.
- Associate of Applied Science (AAS) degree in Nursing or related field.
- Minimum of 1 year of experience; 3 years preferred in case management or community health.
- Strong communication and organizational skills for effective client interaction and documentation.
- Comfortable traveling throughout the Bronx for home visits and community engagement.
- Experience with medically complex populations or behavioral health is advantageous.
