Memory Care Coordinator Overview
- The Memory Care Coordinator oversees the Memory Care program, ensuring effective case management and care coordination for clients.
- Provides direct supervision to staff, ensuring program needs are met and services delivered efficiently.
- Coordinates entitlements, concrete services, and documentation compliance for the Memory Care population.
- Manages referrals, intake, and attendance processes for GUIDE/Respite program clients.
- Ensures timely completion of screens and coordination for GUIDE referral sources.
- Collaborates with the Administrative Manager to maintain efficient day-to-day program operations.
- Provides social work support to Memory Care clients and their caregivers, addressing individual needs.
- Maintains professional competence through attending relevant courses and seminars.
- Works closely with transportation staff to ensure safe and efficient services for registrants.
Memory Care Coordinator Key Responsibilities & Duties
- Conduct initial and ongoing social work assessments for Memory Care and Respite program clients.
- Ensure client attendance aligns with authorization and schedules, resolving participation challenges.
- Supervise staff assignments, reporting performance issues to Clinical Supervisor and Administrative Manager.
- Provide orientation for new staff, interns, and volunteers, ensuring operational readiness.
- Coordinate clinical staff coverage and assignments with the Clinical Supervisor.
- Lead care planning meetings, ensuring person-centered, individualized care plans.
- Facilitate staff in-services and respond to registrant emergencies promptly and effectively.
- Maintain collaborative relationships with external social service agencies and represent the program as needed.
- Evaluate workloads and motivate staff to perform duties efficiently.
Memory Care Coordinator Job Requirements
- Master of Social Work degree with a minimum of 1 year of experience, 2 years preferred.
- Proficiency in bilingual communication is preferred for effective client interaction.
- Strong knowledge of DOH requirements and social work documentation standards.
- Experience in case management, care coordination, and program supervision.
- Ability to lead care planning meetings and develop person-centered care plans.
- Competence in managing referrals, intake processes, and attendance tracking.
- Capability to provide training and orientation for staff and volunteers.
- Proficiency in maintaining professional relationships with external agencies.
