- Provide compassionate, patient-centered care to individuals in hospice, ensuring their physical, emotional, and psychosocial needs are met effectively.
- Develop and implement personalized nursing care plans for patients in end-of-life stages, promoting dignity and comfort.
- Collaborate with interdisciplinary teams to deliver holistic and comprehensive care tailored to patient needs.
- Supervise and evaluate the performance of home health aides and personal care workers in patient care settings.
- Request and manage physician orders to adapt care plans based on evolving patient conditions.
- Conduct thorough assessments and provide expert recommendations to enhance patient comfort and care quality.
- Ensure accurate and timely documentation of clinical data and patient progress in compliance with regulations.
- Maintain adherence to hospice care standards and regulations, fostering a culture of excellence and compassion.
Key Responsibilities & Duties
- Perform comprehensive physical and psychosocial assessments for hospice patients, identifying needs and priorities.
- Develop and implement individualized nursing care plans, ensuring alignment with patient preferences and clinical guidelines.
- Supervise, train, and evaluate home health aides and personal care workers to maintain high-quality care standards.
- Collaborate with physicians and interdisciplinary teams to update and optimize patient care plans as required.
- Provide hands-on interventions and support to patients, addressing physical and emotional needs effectively.
- Educate and support patients and their families, fostering understanding and comfort during challenging times.
- Maintain accurate, timely, and compliant clinical documentation, ensuring transparency and accountability.
- Participate in team meetings and contribute to the development of best practices in hospice care delivery.
Job Requirements
- Associate of Applied Science (AAS) in Nursing or equivalent educational qualification is required.
- Minimum of 2 years of clinical nursing experience, with hospice care experience being preferred.
- Proficiency in clinical data documentation and developing patient care plans effectively.
- Strong communication, teaching, and interpersonal skills to engage with patients and teams.
- Ability to work collaboratively within interdisciplinary teams to deliver comprehensive care.
- Knowledge of hospice care standards, regulations, and best practices to ensure compliance and quality.
- Capability to supervise and evaluate home health aides and personal care workers effectively.
