Job Description
Overview
- Serve as a Diabetes Educator, providing specialized care to members with diabetes.
- Collaborate with healthcare providers to develop personalized diabetes management plans.
- Educate members on diabetes self-management, including lifestyle adjustments and medication adherence.
- Enable members to utilize diabetes monitoring devices effectively.
- Support members in reducing risks of complications and improving overall health outcomes.
- Work within a hybrid schedule, alternating between remote and in-office duties.
- Contribute to quality assurance and improvement processes within the department.
Key Responsibilities & Duties
- Assist members with newly diagnosed diabetes and those with high A1Cs.
- Develop and implement diabetes management plans tailored to individual needs.
- Educate members on healthier food choices, physical activity, and glucose monitoring.
- Provide telephonic support and guidance to members and providers.
- Reduce the risk of complications and emergency visits through proactive education.
- Train members on the use of diabetes management devices.
- Perform additional duties and projects as assigned by management.
Job Requirements
- Valid New York State Registered Nurse (RN) license required.
- Bachelor’s degree in Nursing preferred.
- Diabetes Care and Education Specialist certification required.
- Minimum of three years of diabetes education experience required.
- Strong clinical assessment and patient education skills essential.
- Intermediate computer literacy and proficiency in Microsoft Office preferred.
- Bilingual in Spanish and valid NYS Driver’s license preferred.
- Ability to work flexible hours, including evenings and weekends.
