Registered Nurse – Diabetes Educator

Overview

• Serve as a Diabetes Educator, providing specialized care to members with diabetes.
• Collaborate with healthcare providers to develop personalized diabetes management plans.
• Educate members on diabetes self-management, including lifestyle adjustments and medication adherence.
• Enable members to utilize diabetes monitoring devices effectively.
• Support members in reducing risks of complications and improving overall health outcomes.
• Work within a hybrid schedule, alternating between remote and in-office duties.
• Contribute to quality assurance and improvement processes within the department.

Key Responsibilities & Duties

• Assist members with newly diagnosed diabetes and those with high A1Cs.
• Develop and implement diabetes management plans tailored to individual needs.
• Educate members on healthier food choices, physical activity, and glucose monitoring.
• Provide telephonic support and guidance to members and providers.
• Reduce the risk of complications and emergency visits through proactive education.
• Train members on the use of diabetes management devices.
• Perform additional duties and projects as assigned by management.

Job Requirements

• Valid New York State Registered Nurse (RN) license required.
• Bachelor’s degree in Nursing preferred.
• Diabetes Care and Education Specialist certification required.
• Minimum of three years of diabetes education experience required.
• Strong clinical assessment and patient education skills essential.
• Intermediate computer literacy and proficiency in Microsoft Office preferred.
• Bilingual in Spanish and valid NYS Driver’s license preferred.
• Ability to work flexible hours, including evenings and weekends.
