Job Description

Hospice Care Planner Overview

  • The Hospice Care Planner will evaluate and establish care plans for patients referred for home health services.
  • Facilitate admissions from hospital discharge to home care programs tailored to patient and family needs.
  • Collaborate with physicians, social workers, and case managers to ensure seamless transitions to hospice care.
  • Work on-site at major medical centers, contributing to patient-focused care planning.
  • Engage in marketing and business development efforts to maintain positive relationships within the medical community.
