Hospice Care Planner Key Responsibilities & Duties

Hospice Care Planner Job Requirements

• Current registration as a Registered Professional Nurse in New York State is required.
• Associate of Applied Science (AAS) degree in nursing or related field is mandatory.
• Minimum of two years of experience in hospice, critical care, oncology, or ICU settings.
• Proficiency in physical assessment and patient interviewing skills is essential.
• Strong communication skills, both written and verbal, are required.
