Job Description

Referral Coordinator Overview

  • The Referral Coordinator is responsible for managing and coordinating patient referrals as ordered by healthcare providers.
  • Ensure patients are aligned with primary care providers before initiating referral processes.
  • Collaborate with Patient Care Coordinators to ensure seamless referral operations and patient satisfaction.
  • Maintain structured data fields in electronic medical records for accurate referral tracking.
  • Coordinate referral appointments with external specialists and communicate with patients and providers.
  • Secure insurance authorizations and ensure proper documentation for patient appointments.
  • Participate in daily huddles to update referral statuses for patients scheduled for visits.
  • Provide coverage at alternate locations and work evenings or weekends as required.
  • Adhere to organizational values and contribute to advocacy events when needed.

Referral Coordinator Key Responsibilities & Duties

  • Coordinate all aspects of patient referrals, including scheduling and tracking outcomes.
  • Ensure proper paperwork is provided to patients for appointments as per provider orders.
  • Track referral appointment results and update medical records accordingly.
  • Maintain and update specialty provider lists, including accepted payor sources.
  • Collaborate with healthcare providers and patients to identify suitable specialists.
  • Secure necessary insurance authorizations for referrals and appointments.
  • Participate in daily team huddles to discuss referral statuses and patient needs.
  • Provide coverage at alternate locations and adapt to evening or weekend schedules.
  • Perform additional duties as assigned while maintaining high performance standards.

Referral Coordinator Job Requirements

  • High School Diploma or GED required; experience in medical-related fields preferred.
  • Proficiency in Microsoft Office Suite, Outlook, and internet applications.
  • Strong interpersonal skills to manage sensitive and confidential client information.
  • Ability to work with culturally diverse populations and prioritize multiple demands.
  • Experience in referral coordination and familiarity with electronic medical records.
  • Willingness to work occasional evenings and weekends with advance notice.
