Job Description

Registered Nurse Overview

  • The Registered Nurse will provide and coordinate multidisciplinary health services for older adults in a dynamic Adult Day Care Center.
  • Collaborate with physicians and healthcare teams to implement comprehensive care plans tailored to patient needs.
  • Perform clinical assessments to identify service needs and ensure documentation of findings and care plans.
  • Administer medications, oversee nursing tasks, and ensure compliance with MD orders and care plans.
  • Educate patients and families on disease processes, self-care techniques, and preventative strategies.
  • Monitor patient progress, revise care plans, and communicate changes to the healthcare team.
  • Participate in multidisciplinary meetings to ensure adherence to care plans and report patient progress.
  • Maintain detailed documentation of all services provided and communications with the healthcare team.

Registered Nurse Key Responsibilities & Duties

  • Conduct comprehensive clinical assessments upon intake and quarterly for registrants.
  • Document nursing evaluations, care plans, and recommendations in clinical records.
  • Ensure timely completion and receipt of MD orders, following up as necessary.
  • Administer medications, prefill prescriptions, and provide skilled nursing services.
  • Assign and oversee completion of CAN tasks based on care plans.
  • Coordinate cost-efficient patient care under insurance plans approved by physicians.
  • Utilize diagnosis coding skills to accurately rate registrant acuity.
  • Collaborate with clinical supervisors and medical directors to meet care plan goals.
  • Educate patients and families on health management and preventative care strategies.

Registered Nurse Job Requirements

  • License and current registration as a Registered Professional Nurse in New York State.
  • Bachelor of Science in Nursing (BSN) from an accredited institution preferred.
  • Minimum one year of experience in acute or subacute care preferred.
  • Experience in Adult Care Centers and geriatric care highly desirable.
  • Bilingual proficiency in Spanish and English preferred.
  • UAS certification is a strong advantage.
  • Ability to perform clinical assessments and develop tailored care plans.
  • Strong documentation and communication skills to ensure optimal patient care.
