- The Interim Director of Case Management will lead care coordination and social services programs, ensuring compliance and optimizing patient outcomes.
- Provide strategic leadership for multidisciplinary teams, focusing on patient flow, discharge planning, and resource utilization.
- Collaborate with physicians, nursing leadership, and hospital administration to enhance care delivery and transitions across the continuum.
- Drive initiatives to improve quality outcomes, regulatory compliance, and operational efficiency within the department.
- Serve as a clinical advisor for care management activities, addressing diverse patient populations and payment models.
- Develop and implement policies, procedures, and performance standards to achieve departmental goals.

Interim Director of Case Management Job Requirements
- Bachelor’s degree in Nursing, Healthcare Administration, Social Work, or a related field.
- Active Texas RN license required; CCM or ACM certification preferred.
- Minimum 7 years of healthcare experience with 5 years in leadership roles.
- Experience in hospital-based care management and coordination leadership.
- Strong understanding of regulatory requirements and accreditation standards.
- Proficiency in managing budgets, staffing, and operational initiatives.
- Knowledge of value-based care, utilization management, and population health strategies.
