Job Description

Interim Director of Case Management Overview

  • The Interim Director of Case Management will lead care coordination and social services programs, ensuring compliance and optimizing patient outcomes.
  • Provide strategic leadership for multidisciplinary teams, focusing on patient flow, discharge planning, and resource utilization.
  • Collaborate with physicians, nursing leadership, and hospital administration to enhance care delivery and transitions across the continuum.
  • Drive initiatives to improve quality outcomes, regulatory compliance, and operational efficiency within the department.
  • Serve as a clinical advisor for care management activities, addressing diverse patient populations and payment models.
  • Develop and implement policies, procedures, and performance standards to achieve departmental goals.
  • Monitor quality metrics, productivity, and patient outcomes to identify opportunities for process improvement.
  • Advocate for patients and families, ensuring care plans address clinical, psychosocial, and post-acute needs.
  • Manage departmental budgets, staffing, and financial accountability while supporting organizational initiatives.

Interim Director of Case Management Key Responsibilities & Duties

  • Direct the planning and daily operations of care management services while ensuring regulatory compliance.
  • Lead multidisciplinary teams responsible for case management, social work, utilization management, and discharge planning.

  • Develop and maintain relationships with payers, post-acute providers, and community resources.
  • Evaluate alternatives to acute care considering clinical appropriateness and financial impact.
  • Monitor departmental performance metrics, identifying opportunities for improvement.
  • Manage staffing, operational, and financial issues within the department.
  • Support initiatives involving value-based care, utilization management, and population health.
  • Advocate for patients and families, ensuring comprehensive care plans are implemented.

Interim Director of Case Management Job Requirements

  • Bachelor’s degree in Nursing, Healthcare Administration, Social Work, or a related field.
  • Active Texas RN license required; CCM or ACM certification preferred.
  • Minimum 7 years of healthcare experience with 5 years in leadership roles.
  • Experience in hospital-based care management and coordination leadership.
  • Strong understanding of regulatory requirements and accreditation standards.
  • Proficiency in managing budgets, staffing, and operational initiatives.
  • Knowledge of value-based care, utilization management, and population health strategies.
  • Ability to lead multidisciplinary teams and drive process improvements.
