Case Management Supervisor Overview

  • The Case Management Supervisor oversees healthcare case management staff, ensuring quality care delivery and adherence to policies and standards.
  • Collaborates with multidisciplinary teams to achieve optimal health outcomes for patients through monitoring and evaluation.
  • Provides direct case management services to diverse populations, addressing specific needs and resolving patient concerns effectively.
  • Develops and supervises patient-focused care plans, ensuring input from care teams and patients.
  • Conducts quality assurance and improvement activities to enhance service delivery and patient satisfaction.
  • Represents the organization in external venues to promote services and foster partnerships.
  • Ensures compliance with local, state, and federal regulations, maintaining accurate records and documentation.
  • Participates in outreach and recruitment activities to promote agency services effectively.

Case Management Supervisor Key Responsibilities & Duties

  • Supervise healthcare case management staff, providing training, coaching, and performance reviews to ensure service quality.
  • Coordinate patient care activities, including assessments, care planning, and crisis intervention.
  • Monitor team performance to meet service goals and quotas set by the department.
  • Provide direct case management services, collaborating with care teams and families to achieve positive health outcomes.

Case Management Supervisor Job Requirements

  • Bachelor’s degree in health, nursing, public health, or related field required; advanced degree preferred.
  • Minimum of four years of relevant experience, with one year of supervisory experience preferred.
  • Experience working with diverse populations, including individuals with mental health conditions and chronic illnesses.
  • Proficiency in software applications such as Word, Excel, and healthcare databases.
  • Strong communication skills and ability to work collaboratively in team settings.
  • UAS-NY certification required within one month of hire.
  • Flexibility to work evenings and weekends as needed.
