Director Case Management Overview

  • The Director Case Management oversees comprehensive case management programs, ensuring alignment with the hospital's mission and patient needs.
  • Provides leadership to case managers, social workers, and coordinators, fostering professional development and operational excellence.
  • Designs and implements services that integrate continuous quality improvement tools for optimal patient outcomes.
  • Assesses and plans services tailored to patient and family needs, ensuring effective communication and coordination.
  • Collaborates with multidisciplinary teams to enhance utilization review and discharge planning processes.
  • Leads initiatives to improve case management systems, aligning with organizational goals and regulatory requirements.
  • Ensures compliance with industry standards and integrates evidence-based practices into case management operations.

Director Case Management Key Responsibilities & Duties

  • Develop and supervise staff within the case management department, ensuring effective operations and professional growth.
  • Coordinate services across departments, utilizing quality improvement tools to enhance patient care.

  • Oversee utilization review processes, ensuring compliance with regulatory standards and criteria.

Director Case Management Job Requirements

  • Bachelor of Science in Nursing (BSN) or related field required.
  • Minimum of 5 years of experience in case management roles, with leadership experience preferred.
  • Current CCM certification or ability to obtain within one year preferred.
  • Proficiency in Milliman and InterQual criteria for utilization review processes preferred.
  • Strong analytical skills for data evaluation and problem-solving in case management operations.
  • Experience with basic to intermediate computer skills, including case management software.
  • Knowledge of continuous quality improvement tools and evidence-based practices in healthcare.
  • Licensed clinician in your state or equivalent qualifications required.
