Overview
- Lead and manage elder justice services within a skilled-nursing facility environment.
- Provide trauma-informed counseling, advocacy, and case management for older adults.
- Collaborate with multidisciplinary teams to develop care and safety plans for clients.
- Advocate for clients within various systems of care to ensure their needs are met.
- Develop and deliver professional training programs on elder mistreatment and trauma-informed care.
- Contribute to grant reporting and program development initiatives.
- Utilize a client-centered, anti-oppressive approach in all aspects of service delivery.
- Maintain accurate case records and ensure compliance with organizational standards.
- Participate in ongoing professional development and training activities.
Key Responsibilities & Duties
- Conduct safety assessments and develop individualized safety plans for clients.
- Provide crisis intervention, case management, and supportive counseling services.
- Collaborate with attorneys, healthcare professionals, and social workers to deliver comprehensive care.
- Create and deliver training sessions for professionals on elder justice topics.
- Advocate for clients within healthcare and legal systems to address their needs.
- Assist in the preparation and submission of grant reports.
- Utilize computer systems to manage case records and documentation.
- Participate in team meetings and contribute to program development efforts.
- Maintain clear and concise communication with clients and team members.
Job Requirements
- Master of Social Work (MSW) degree required; LMSW or LCSW certification preferred.
- Minimum of 2 years of experience in geriatrics, elder abuse, or healthcare.
- Proficiency in trauma-informed care approaches and elder justice practices.
- Strong interpersonal skills and ability to present information effectively.
- Experience in program development and professional training delivery.
- Basic computer skills for case record management and documentation.
- Ability to work independently and within multidisciplinary teams.
- Proficiency in a language other than English is helpful but not required.
- Commitment to anti-oppressive, client-centered service delivery.
