Job Description
Responsibilities:
- Meet with members that are interested in enrolling in our Plan
- Review their NYIA and create a tasking tool to determine level of service/hours we can provide
- Create the PCSP (person centered service plan) which is reviewed with the member prior to enrollment.
Requirements:
- RN Degree in the State of NY
- Reliable Vehicle for Driving
- Prior knowledge of UAS
