Case Manager MLTC

Responsibilities:

• Responsible for assessing potential new members for eligibility for the program and for explaining the program to them and their family. 
• Responsible for knowing require outreach activities within the community and the assessment of eligibility and safety of potential members.
• Collaborate with other members of the Intake/Outreach team and the clinical team to assure the appropriate provision of services to new members.
• Communicate with potential members, family, referral sources and physicians to assure a seamless transition to managed long term care.

Qualifications:

• Must be a self-assured professional with a current New York State Registered Nurse License required.
• Bachelor’s degree in Nursing required.
• One (1) year experience in an acute care setting required.
• Must have UAS- NY experience. Home care and management experience preferred.
• MS Office required.
