Job Description
Responsibilities:
- Offer direct support and coaching to participants, helping them navigate through the program's requirements and services.
- Facilitate access to resources and referrals, addressing individual needs related to employment, health, and housing stability.
- Conduct regular follow-ups to assess progress, update plans, and ensure engagement.
- Document interactions and maintain detailed records of participant progress.
- Collaborate with community organizations and partners to enhance service offerings and support network for participants.
- Actively participate in team meetings and training sessions to stay informed of program updates and best practices.
Minimum Qualifications:
- Minimum of two (2) years of experience working with populations facing homelessness and disabilities, which may include mental illness, substance use disorders, medical conditions, and cognitive impairments. Experience with crisis intervention preferred.
- Bachelor’s degree in Social Work, Counseling, or Psychology.
