Social Work Case Manager

Duties:

• Reporting to the Supervisor of care management this position will provide case management to our members including medical, social, psychological, physical and spiritual needs by developing, implementing and monitoring the care plan, in conjunction with the PCP, caregivers and other team members, to help meet the member's needs.

Qualifications:

• BGD: LMSW – Minimum of three years' experience in MLTC- Medicare managed care experience required.
• Working knowledge of utilization management processes and case management also required.
• Knowlege of federal and State Managed Care regulations, InterQual, and local Medicare coverage guidelines.
• Knowledge of windows, Word and Excel.
• Good written and verbal communication skills.
