Job Description

MLTC: Occupational Therapist (will consider a Physical Therapist)

Position Overview:

  • This is a CASE MANAGER position – HYBRID (2 days Remote & 3 Days Office)
  • NO Hands-on Care
  • Setting: Office and Work-from-Home (HYBRID)
  • After probation is successfully completed, they can be remote 2 days per week and 3 days in the Queens Office

Responsibilities:

  • Assist in the clinical management, oversight, and supervision of rehabilitation services in the MLTC, MAP, and ESNP programs
  • Review clinical rehab documentation from outside vendors and providers to ensure members receive appropriate service
  • Review requests for all DME in the home for appropriateness
  • Provide on-site education to nurse managers and VPs regarding rehab's role in member care
  • Conduct field visits as required to assess high-end DME needs and home safety assessments
  • Ensure compliance consistent with health rules of the New York State DOH

Qualifications:

  • Master’s degree in healthcare or a related field
  • Current New York State Occupational Therapist license
  • Two to four years of experience as an Occupational Therapist
  • Excellent computer, organizational, and interpersonal skills
  • Ability to multi-task in a fast-paced environment
  • Requires home visits, as needed
