Director of Nursing

Responsibilities:

• Supervises all direct patient/family care staff, including Nurses, Social Workers, Home Health Aides and Spiritual Care Coordinators.
• Supervises coordination with Medical Director and attending physicians during the initial Plan of Care development at admission and as the Plan of Care is updated.
• Ensures that all patient/family services comply with Federal, State and local regulatory requirements in accordance with professional standards.
• Participates in the development, implementation and evaluation of policies and procedures for patient/family services.
• Actively participates in the selection and evaluation of all direct patient/family care staff.
• Coordinates the development, implementation and review of the Interdisciplinary Plan of Care for each patient/family.
• Maintains the Hospice patient clinical records, hard copy and computer generated records for each service provided to each patient according to established regulatory standards and agency policy.
• Conducts the interdisciplinary team meetings and maintains appropriate documentation, attendance and all changers within the patient Plan of Care.
• Ensures the 24-hour availability of nursing services to patients/families creates and coordinates the "on-call" Schedule.

Requirements:

• RN, Bachelors' Degree
• Seasoned leadership experience
• 4-6 years in a hospice, homecare or long term care setting
