Case Manager MLTC

Responsibilities:

• The role of the Nurse Care Manager plays a critical role in collaborating with the member and/or family member to maintain the most independent living situation possible.
• Ensure consistent care along the entire health care continuum by assessing and closely monitoring member’s needs and status.
• Will be responsible to authorize covered services and coordinate care regardless of payer.
• The Nurse Care Manager will collaborate and communicate with member/family/caregivers, primary care practitioners, and the interdisciplinary team.

Qualifications:

Must possess a current NYS RN License and registration. Bachelor of Science degree in Nursing required with a minimum three (3) years of RN experience. Clinical expertise in geriatrics preferred; managed long term care experience preferred. Bilingual skills may be required as determined by operational needs.
