Registered Nurse

Responsibilities: 

Provides nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the interdisciplinary Plan of Care. Supervises Home Health Aides. Supports the Provider Relations and Quality Improvement activities of the organization.

• Receives patient assignments from the Hospice Nurse Coordinator
• Assumes responsibility for a patient/family that includes assessing, planning, implementing and evaluating care within the first 24 hours of patient/family admission.
• Ensures that the plan of care reflects the patient’s current conditions and needs based upon the initial and all subsequent assessments.
• Nursing visit notes address and reflect those issues that are on the plan of care. New conditions or changes are reflected on the Plan of care through the hospice IDT plan of care update. Assumes leadership in all modifications of care planning and ensures all interventions are implemented as planned/prescribed.
• Initiates communication with attending physicians, other hospice staff members, contract providers, and other agencies as needed to coordinate optimal care and use of resources for the patient/family.

Qualifications: 

• 1+ year of Nursing experience 
• Associate's and/or Bachelor's Degree in Nursing 
• NYS Registered Nurse (RN) license 
• Solid assessment, clinical, and documentation skills  
• Patient-oriented
