Job Description
ROLE SUMMARY
The RN is part of a multidisciplinary team of a patient-centered medical home (PCMH) providing direct patient care and supervising nursing personnel in the assigned clinic area by performing the following duties.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Provides patient education, routine and urgent treatment in primary and specialty care departments within the scope of practice of NYS RN licensure and according to UHC policies and procedures.
- Assists providers in responding to desktop messages and completion of RX refills for patients.
- Provides translation for providers when needed.
- Supervises the Licensed Practical Nurse (LPN). In collaboration with Team Supervisors, supervises Medical Assistants. In the absence of the Operations Manager supervises Health Coaches and Team Supervises.
- Prepares and completes annual performance appraisals and competency evaluations for Supervised staff on the Specialty Team and Primary Care LPNs.
- Functions as a Case Manager by facilitating the delivery of individualized and coordinated care with input from members of the healthcare team for patients with Complex Care needs.
- Develops and updates a patient’s individualized care plan that incorporates treatment goals barriers to goals and self- management plan post ER visit, hospitalization and /or Provider referrals.
- Provides recommendations to administration for improvement of patient services and clinical work processes.
- Administers injections as per electronic orders, and performs treatments as ordered per UHC policies and procedures.
- Performs phlebotomy as ordered by UHC Medical Staff.
- Assists with all pre-authorizations and referrals requiring communication of clinical information as needed.
- Reviews abnormal labs and follows protocols as determined by UHC Medical Staff.
- Directs and assists ancillary nursing personnel in opening center and preparing patients for provider visits.
- Works closely with Providers and Medical Assistants by attending daily team huddles to discuss and pre-plan patient care.
- Works collaboratively with Providers and Medical Assistants in the management of scheduling conflicts with patients.
- Responds to patient portal requests in collaboration with UHC medical staff.
- Initiates pre-op orders as requested by surgical office for patients scheduled for surgery and in preparation for medical clearance for procedure, by their PCP.
- Provides direct patient teaching; performs medication reconciliation with patients and reports and records information of patient’s responses to therapy.
- Updates City Wide Immunization Registry as needed.
- Triages all new patients and pediatric walk-ins to the Health center and performs phone triage of all sick calls routed from the call center.
- Initiates and completes medical and social history intake of new patients.
- Maintains availability and readiness of all equipment and supplies in the assigned areas.
- Orders medical and non-medical supplies as needed.
- Assists in hiring and orientation of new personnel.
- Coordinates staffing schedules for clinical work areas.
Qualifications
- New York State Licensed Registered Nurse; BSN preferred
- 3 years ambulatory experience preferred / Minimum 1 Year
- Strong interpersonal, oral, written communication and collaborative skills.
- Computer literate.
- Language: Spanish bi-lingual
- Knowledge of EMR a plus
