RN Manager – Case Management

We’re working with a respected acute-care hospital in El Paso, Texas, seeking an experienced RN Case Management Leader to step into a visible and impactful management role. 🔑 Key Responsibilities

Facilitate effective patient care throughout hospitalization, focusing on appropriate discharge planning

Conduct comprehensive assessments of patient needs, resources, and support systems

Supervise case managers and social workers in providing exceptional, patient-centered care

Oversee utilization reviews and ensure appropriate resource use

Maintain proactive communication with providers and third-party payers regarding treatment plans and patient status

Act as a liaison for patients and families, advocating for their healthcare needs and ensuring access to community services

Coordinate post-discharge services, including DME, home health, and placement

✅ Requirements

Bachelor’s Degree in Nursing required

Current RN license in Texas (or compact)

2+ years of supervisory or management experience in case management within an acute care hospital setting

Strong understanding of discharge planning, care coordination, and payer communication
