Job Responsibilities

The Nurse Director of Care Management is responsible for Care Coordination, Utilization Review, Case Management, and Discharge Planning departments. Key responsibilities include:

·        Provide clinical leadership for dedicated team of Case Managers and Social Workers

·        Ensures compliance with federal, state, and accreditation requirements (e.g., CMS, Joint Commission)

·        Collaborates with physicians, nursing, and other departments to ensure safe, timely, and appropriate patient care transitions

·        Oversee the development and execution of individualized care plans

·        Monitors utilization data and implements strategies to reduce length of stay and prevent readmissions

·        Manages departmental budgets and resources effectively

·        Provides staff training, development, coaching, and performance evaluations

·        Engages with external agencies, payers, and community resources to support patient needs

Candidate Qualifications

The ideal Nurse Director of Care Management has:

·        Bachelor’s Degree in Nursing (BSN)

·        At least 3 years of Case Management leadership experience in acute care setting

·        Demonstrated expertise in leading evidence-based practices and process improvement initiatives
