Overview
- Serve as a vital link between medical services and program residents, ensuring comprehensive nursing care and adherence to OPWDD regulations.
- Provide direct nursing care, perform routine medical procedures, and develop individualized nursing care plans for residents.
- Supervise LPNs, AMAPs, and Direct Support Professionals in nursing tasks and activities to ensure quality care.
- Conduct regular nursing assessments and coordinate acute and ongoing health services for residents.
- Maintain accurate medical record documentation and ensure compliance with state and federal regulations.
- Provide training to staff on health measures, medication administration, and client-specific medical issues.
- Participate actively in interdisciplinary team meetings and contribute to care coordination efforts.
- Utilize electronic health record systems for documentation and communication with external service providers.
Key Responsibilities & Duties
- Perform nursing assessments and develop individualized care plans updated annually or as conditions change.
- Coordinate health services and monitor residents' health and treatment efficacy.
- Supervise and evaluate LPNs and direct support staff, ensuring adherence to nursing standards.
- Provide training on health measures, communicable disease control, and client-specific medical issues.
- Ensure proper documentation of training curriculums, attendance, and testing results.
- Conduct annual clinical performance evaluations for unlicensed staff in medication administration and other procedures.
- Maintain on-call availability for backup support and emergency situations.
- Collaborate with interdisciplinary teams to enhance care coordination and service delivery.
Job Requirements
- Bachelor of Science in Nursing (BSN) with active RN licensure.
- Minimum of 1 year of nursing experience, preferably with OPWDD or MRDD populations.
- Proficiency in electronic health record systems and care coordination platforms.
- Strong knowledge of OPWDD regulations and compliance requirements.
- Ability to supervise and train staff in nursing tasks and health measures.
- Flexibility in scheduling and availability for occasional backup on-call responsibilities.
