Overview
- Join a dedicated healthcare team providing inpatient rehabilitation and stabilization services in a 32-bed facility for individuals with chemical addiction.
- Work as a Registered Nurse in a permanent, on-site role with a focus on patient care and medical case management.
- Collaborate with interdisciplinary teams to address clients' medical and psychosocial needs, ensuring comprehensive care delivery.
- Provide health education and advocate for client health preferences and goals, fostering positive health outcomes.
- Utilize your expertise to conduct health and vitals checks, ensuring optimal patient care and safety.
- Contribute to maintaining accurate medication records and controlled substance logs, ensuring compliance and accountability.

Key Responsibilities & Duties
- Act as primary medical case manager for assigned clients, managing health needs and coordinating with nursing leadership and team members.
- Conduct health and vitals checks, ensuring accurate documentation and optimal client outcomes.
- Collaborate with physicians, pharmacies, and treatment collaterals to address clients' medical and psychosocial needs.
- Maintain medication administration records and controlled substance logs, ensuring compliance and medication supply adequacy.
- Provide health education to clients, supporting their health goals and advocating for their preferences.
- Participate in treatment team meetings and case conferences, sharing findings and contributing to care planning.
- Deliver emergency first aid and CPR assistance as needed, ensuring immediate response to critical situations.
- Engage in related duties as assigned, contributing to the overall success of the healthcare team.
Job Requirements
- Bachelor of Science in Nursing (BSN) and active RN license required.
- Minimum of 1 year of nursing experience; 3 years preferred.
- Strong knowledge of medical case management and inpatient rehabilitation practices.
- Proficiency in maintaining medication records and controlled substance logs.
- Ability to collaborate with interdisciplinary teams and external treatment providers.
- Excellent communication skills for client education and advocacy.
- Availability to work Wednesday to Saturday, 9 AM to 7 PM, including weekends.
